The Rail Accident Investigation Branch (RAIB) has launched an investigation after a one-tonne steel pile was left on a railway line in Lancashire.
A railway maintenance vehicle stopped short of the large steel structure near Kirkham lsat month (15th May).
RAIB said the pile had been left there on completion of earlier engineering works.
A preliminary examination has been carried out and a safety report is due to be published in the near future.
