A Derbyshire-based construction company has been fined after a worker fell down a service shaft while dismantling falsework at a building in Leicester.
David Ashley Construction Limited, of Lydford Road, Alfreton, Derbyshire pleaded guilty to breaching Regulation 4(1) of the Work at Height Regulations 2005.
It was fined £20,000 with £1,776 in costs.
Leicester Magistrates' Court heard how the 36-year-old worker from Romania was dismantling falsework (a form of temporary structure) at the under-construction building at Fletcher Development, De Montfort University on 15 June 2015.
However, whilst carrying out the work, he fell 13 metres down a service riser shaft.
A Health and Safety Executive (HSE) investigation into the incident discovered there was an unsafe system of work in place, as well as inadequate supervision of workers.
